After a hospital stay, the gap between discharge and full recovery is where most setbacks happen. We bridge that gap. Your therapist comes to your home, builds a recovery plan around your space and your schedule, and works alongside or in place of home health. Sessions are one-on-one, unhurried, and focused on getting you back to the routine you had before the hospital, not just getting you to baseline.
We treat patients recovering from a wide range of hospital stays: orthopedic surgeries, cardiac events, pneumonia, infections, falls, and general deconditioning after extended bed rest.

Post-surgical recovery is a window with a clear opening and closing. Acting early matters. We offer pre-hab — physical therapy before surgery to help patients build strength and prepare their bodies for an upcoming procedure such as a total knee replacement — and structured in-home rehab that begins as soon as you return home.
We support recovery from joint replacements (knee, hip, shoulder), rotator cuff repairs, spinal surgeries, abdominal surgeries, and most other post-operative situations where mobility and strength need to be rebuilt.

Falls Prevention

One in four adults over 60 will experience a serious fall this year, and most of those falls are preventable. Our falls prevention program assesses balance, strength, gait, vision, medication interactions, and home environment, then builds a targeted plan around the specific risk factors we identify. We also work with clients who have already fallen, helping them rebuild confidence and prevent the next one.
The program runs in your home, not in a clinic, because the home is where falls happen. We can adjust rugs, recommend grab bars, identify lighting issues, and rehearse the specific transitions (out of bed, into the bathroom, down the stairs) where falls most often occur.

Family and Caregiver Training

Most caregiving is done by family members who were never trained for it. That training gap is where injuries happen, both to the patient and to the caregiver. We teach the people closest to the client how to help safely and effectively.
Topics covered depend on the situation but typically include: safe transfers (bed to chair, chair to walker, into a vehicle), positioning to prevent pressure sores, how to spot and respond to dizziness or instability, body mechanics so the caregiver does not injure their own back, and how to encourage progress without pushing too hard.
Available as part of an ongoing care plan or as a standalone session for families who just need a few hours of focused training.

Strength Coaching
Personalized strength training for older adults who want to stay strong.

Strength loss after 60 is not inevitable, but it is the default if no one intervenes. We work with clients who are not recovering from a specific injury but want a structured, ongoing strength plan that matches their body, their goals, and their home environment.
Sessions cover assessment of current strength baseline, progressive resistance work using equipment that fits the home, mobility and posture corrections, and education on how to keep training between sessions. Many clients transition into strength coaching after finishing rehab; others come to us preventively, before the first injury or fall.

The hardest part of any health plan is sticking with it after the initial enthusiasm fades. We schedule regular accountability check-ins, set realistic goals you can actually measure, and adjust the plan when life gets in the way. We are not pushy and we are not your drill sergeant. We are the trained second opinion who notices when you are slipping and helps you get back on track without judgment.
Lifestyle education covers sleep, hydration, daily activity targets, posture during everyday tasks, and how to integrate movement into a routine you already have.

Nutrition Management and Advice

Nutrition for older adults is different from nutrition for the general population. Recovery, medication interactions, protein needs, hydration, and appetite changes all play a larger role. We provide practical, plain-language nutrition guidance focused on eating to support strength, recovery, and energy.
This is general nutrition education provided within a physical therapy scope of practice. We are not registered dietitians and we do not write medical nutrition therapy plans. For complex medical nutrition issues, we will refer you to a registered dietitian nutritionist (RDN).

Equipment Training

A walker used incorrectly is more dangerous than no walker at all. The same is true for canes, braces, CPM machines, compression devices, and most home recovery equipment. Hospitals and surgical centers send patients home with equipment and a quick demonstration; we follow up at home to make sure the equipment is set to the right height, used in the right situations, and not creating new problems.
We also help families decide whether the equipment they have is the right fit, or whether something different (or simpler) would serve them better.
