Do You Need a Doctor’s Referral for Physical Therapy in Kentucky or Indiana?

If you have been putting off physical therapy because you think you need a doctor’s order first, here is the short version: in both Kentucky and Indiana, you don’t. You can hire a licensed physical therapist directly — no referral, no waiting weeks for a physician appointment, no permission slip.

This single misunderstanding stops more people from getting care than almost anything else. A parent is recovering slowly after a hospital stay, a fall has shaken the whole family’s confidence, and everyone assumes the first step is booking a doctor’s visit to “get the referral.” It isn’t. Here is how direct access actually works, and what it means for families across Louisville and Southern Indiana.

The Short Answer: No Referral Needed in Kentucky or Indiana

Kentucky and Indiana are both direct-access states. That means state law lets a licensed physical therapist evaluate you and begin treatment without a referral or prescription from a physician. You can reach out to a PT the same way you would reach out to a dentist or an eye doctor — directly, on your own timeline.

For families in our nine-county service area — Jefferson, Bullitt, Hardin, Spencer, Shelby, and Oldham counties in Kentucky, and Clark, Floyd, and Harrison counties in Indiana — that means care can start this week instead of after a chain of appointments.

What “Direct Access” Actually Means

Direct access does not mean physical therapists work in isolation. It means the law recognizes that a licensed PT is trained to evaluate you safely and knows when something belongs with another provider. In practice:

  • You can be evaluated and treated right away. Your therapist performs a thorough assessment on the first visit and, if physical therapy is appropriate, begins care the same day.
  • Your therapist screens for anything outside PT scope. Part of that first evaluation is making sure your symptoms are a good fit for physical therapy. If something needs a physician’s attention first, we tell you and help you get there.
  • You stay in control of your care. No gatekeeper decides whether you are “allowed” to start. You decide, and your therapist’s clinical judgment guides the plan from there.

Why So Many People Think They Need a Referral

The referral myth is stubborn for a few understandable reasons. For decades, direct access wasn’t the law everywhere, and old habits linger. But the biggest reason is insurance:

  • Insurance plans often require a referral to pay. That is a billing rule set by the insurer, not a law about whether you are allowed to receive care. The two get confused constantly.
  • Clinics ask for the paperwork up front, because chasing an authorization after the fact is a headache for them. So patients hear “we need your referral” and assume it is a legal requirement.

Because Kentuckiana Mobile Therapy is a cash-based practice, the insurance referral requirement simply doesn’t apply to us. There is no insurer in the room requiring a physician’s sign-off before your therapist can help you. You call, we evaluate, and care begins.

When a Physician Referral Still Makes Sense

We would rather be straight with you than oversimplify. There are still situations where looping in a doctor is the right move:

  • Post-surgical protocols. After a joint replacement or other procedure, your surgeon usually has specific precautions and timelines. We follow those protocols closely and coordinate directly with the surgical team when we deliver in-home post-surgical rehab.
  • Medicare home health. If you qualify for Medicare-covered home health after a hospital stay, that benefit does require physician involvement — use it, and many families continue privately with us when those visits end but recovery isn’t finished.
  • Coordinating your bigger picture. If you want your primary care doctor kept in the loop, we are glad to share notes and progress so everyone on your care team is working from the same page.

The point is that a referral can be useful, but for most people in Kentucky and Indiana it is not a requirement standing between you and your first appointment.
